The title of your post should be the character name. For example, in this case, you would be reading the profile of a character named "Template". Please for the love of Jack Daniels in a glass don't post as a reply on this posting.

Name: (This should be your character's full name. Make it realistic, please. If you really want to be known as Princess Plum of the Sugar People, you better have an excellent reason as to why that is your legal name. Surnames are not a necessity for vampires and fey, but humans and lycanthropes usually have last names. Use your better judgment.)

I won't accept a character that turns into a neon blue wolf or a vampire with purple polka-dot eyes. Weretigers get special dispensation, because for some reason they're weird colors. Go figure.)

Character Concept/History: ( Interesting characters have back story. What’s your character hiding. We don't need every detail of your character's life. However, if you have a special back story that you really want to put down, this is the place to do it. Bare minimum, this should cover why your character is in Chicago(did they grow up here, or have they come here for a specific reason?), how they became what they are (at least for those characters who aren't born into their species), and any events that are integral to their character (for example, if their parents died when they were young, or if they spent a lot of time moving around, or maybe if something got amputated - kinda important, right?). Use your discretion otherwise.
Angsty backgrounds are nice and all and may contribute to the characters inherent flaws and strengths but, not everyone can be an orphan who had their whole family slaughtered before their eyes. Even if your application is excellent, this particular section can get you declined if it is too stereotypical! You can be as creative as you like, but some semblance of "boring" normality is actually a good thing for this site.)
